From Research to Certified Clinical TMS Therapy on a Speedgoat Target Machine Platform

The medical technology startup sync2brain, based in Tübingen, Germany, is developing advanced therapies for neuropsychiatric disorders such as depression and OCD that could significantly improve patient outcomes.

"Our solution has the potential to deliver faster, more effective, and highly personalized TMS therapies for patients suffering from depression or OCD. Speedgoat has played a critical role in taking this technology from concept to a clinically deployable reality."

With its brain oscillation state sensor device (bossdevice®), the company enables a more personalized approach for transcranial magnetic stimulation (TMS). The bossdevice continuously measures the brain’s natural activity in real time and delivers stimulation precisely when the brain is most receptive. By synchronizing stimulation with the brain’s own rhythms, the therapy becomes personalized, and through application of suitable protocols, potentially more efficient and effective.

The complete TMS research and therapy setup consists of three core components: EEG-based acquisition of patient brain data, sync2brain’s dedicated computing platform (bossdevice® MEDICAL™ system) for signal processing, and the TMS stimulator that delivers precisely timed therapy.

For optimal results, the data processing and triggering must run in real time and with absolute reliability. As stimulation windows are measured in milliseconds, the system requires seamless data acquisition, analysis, and signal output, with the components operating as a tightly integrated, deterministic real-time platform capable of supporting clinical deployment.

Challenge | High Costs from Separate Medical Devices for Research and Embedded Deployment

sync2brain has extensive domain knowledge in advanced software and algorithms for real-time detection of brain states. However, turning this capability into a clinically deployable system was technically and economically challenging.

The solution needed real-time deterministic computing capable of processing EEG signals, executing data processing algorithms, and triggering stimulation within millisecond time windows. At the same time, maintaining separate platforms for research and a production-ready medical device would have significantly increased development costs and complexity.

Moving from research to a production-ready device also introduced substantial risk, where any latency, jitter, or instability could compromise system performance and clinical viability.

For a young company with limited resources, navigating the complexity of certification and compliance in the heavily regulated medical sector posed an additional barrier to market readiness.

Solution | A Unified Platform for Research and Medical Deployment

To bring its solution from research into clinical practice, sync2brain partnered closely with Speedgoat, using a Speedgoat real-time test system as the common foundation across development and deployment. This approach enabled the same system to support early-stage research as well as a certified medical-grade device, ensuring continuity throughout the entire lifecycle.

By avoiding platform changes between research and clinical use, sync2brain could transfer innovations directly into patient-ready therapy without costly redesigns. The result is a streamlined path from lab to clinic, significantly accelerating time-to-market while meeting all regulatory and quality requirements.

In addition, sync2brain leverages a MATLAB® and Simulink®-based software environment to develop and refine its algorithms for brain-state detection and stimulation timing. With interfaces to its open and extensible software framework, researchers can adapt and validate new therapy approaches directly on the system. This tight integration between modelling, real-time execution, and hardware enables rapid iteration and continuous advancement of brain-state dependent therapy.

The overall solution is built around sync2brain’s bossdevice® RESEARCH™ and bossdevice® MEDICAL systems, both powered by Speedgoat real-time test systems. These systems integrate seamlessly with EEG amplifiers and TMS stimulators from leading vendors, enabling precise, personalized brain stimulation in both research and clinical settings.

Result | Accelerated Certification, Deployment, and Innovation on a Continuous Platform

The collaboration between sync2brain and Speedgoat demonstrates how a close development partnership can turn a complex, timing-critical concept into a clinically viable solution. By building on a shared real-time platform, sync2brain was able to accelerate development, reduce risk, and establish deep technical expertise across both research and medical domains.

To support the transition to a certified product, Speedgoat provided certification services, including an ISO 13485-compliant test setup for the bossdevice, while sync2brain retained full ownership of the certification process. This approach ensured compliance with key medical standards such as IEC 60601, while allowing sync2brain to build up internal know-how and maintain control over its product.

The bossdevice MEDICAL solution is currently awaiting final regulatory approval. Speedgoat supports this next phase with small-batch production of fully tested systems, ensuring consistent performance and quality for early deployments.

Looking ahead, the continued evolution of sync2brain’s solutions for personalized TMS therapy may lead to even more integrated systems combining EEG acquisition, real-time processing, and stimulation. Such advancements bring new technical challenges, including system integration and miniaturization—areas where Speedgoat continues to contribute with its real-time expertise and test system capabilities.
